HCP’s current system, along with third-party tools like Pulse8, created a fragmented user experience for clients. With limited integration, users are forced to navigate multiple platforms to identify gaps in patient care. This lack of seamless connectivity hampers workflow efficiency and increases the manual effort required for data management.
Transitioning from Legacy Systems to the Cloud
This was done by creating a modern, custom web application that will serve as the foundation for HealthCare Partners’ cloud-based solutions. The web portal will enhance gap closure rates while minimizing disruption to providers. By streamlining efforts across documentation, coding, and quality, the platform will eliminate repetitive and unnecessary information requests, allowing providers and their staff to work more efficiently. I led a series of discovery activities, including persona development, journey mapping, and prototyping to achieve the desired outcome.

I’m also building proprietary technology that will scale with HCP’s needs, ensuring a seamless, cloud-powered experience that improves usability and enhances overall efficiency.

I’ve developed a customized web application for HealthCare Partners that enhances the user experience for both physicians and healthcare consumers, streamlining workflows and improving population health management. As a result, the platform has successfully onboarded a higher number of physicians and patients, contributing to a 20% increase in revenue.
